儿童Ⅱ期人工晶状体植入术两种手术方式的对比研究

目的探讨儿童无晶状体眼Ⅱ期人工晶状体植入术的两种手术方式的效果。方法3．5～9岁儿童白内障术后无晶状体眼58例（71眼），随机分成2组：A组36眼，行后囊切开、前段玻璃体切除及后房人工晶状体植入；B组35眼，单纯行后囊切开及人工晶状体植入。结果术后3月，矫正视力〉10．05者，A组30跟（83．33％），B组30眼（85．71％）；≥0．3者，A组21眼（58．33％），B组20眼（57．14％）。脱盲率及脱残率2组之间差异均无显著性（P〉0．05），但两组分别与术前相比差异均有统计学意义（P〈0．01）。两组术后各种并发症之间差异也无统计学意义。结论儿童无晶状体眼Ⅱ期人工晶状体植入术的两种术式均是安全的、有效的，单纯的后囊切开更符合患儿的生理特点。

The contrasting study of secondary intraocular lens implantation in children with two surgical ways

Objective To observe the surgical effect of secondary intraocular lens （IOL） implantation in children with two different suegical ways. Methods The postoperative aphakia eye of cataract were 58 cases （71 eyes） in children whose ages ranged from 3. 5 to 9 years old They were randonly divided into two groups：Group A were 36 eyes and group B were 35 eyes Group A was treated with capsulotomy and anterior vitrectomy and IOL implantation; Group B was only treated with capsulotomy and IOL implantation. Results Three mouths after surgery, corrected visual acuity ≥0.05 was in 30 eyes （ 83.33% ） in group A and in 30 eyes （85.71%） in group B corrected visual acuity ≥0.3 was in 21 eyes （58.33%） in group A and in 20 eyes （57.14%） in group B. There was no statistical difference between two groups. But each of group was statistical difference between preoperative and postoperative. The complications of two groups were no statistical difference. Conclusion Secondary IOL implantation is a safe and effective method in children with aphakia by the two different surgery technique. It is suitable for physiological character of children with simple capsulotomy and IOL implantation in group B.
