bcl-2表达和淋巴结转移可作为皮肤鳞癌独立预后指标

目的寻找皮肤鳞癌有效的预后指标。方法选取32例皮肤鳞癌患者的病理检验蜡块,记录肿瘤大小、淋巴结转移情况;运用TUNEL染色和形态学方法结合,统计凋亡指数、bcl-2和p53表达情况, 随访患者1、2、3、5年生存情况,用SPSS11．0统计有效的预后指标。结果肿瘤淋巴转移以及病理分级与 3、5年生存率均相关(P值分别为0．026、0．037),肿瘤大小与其不相关(P>0．05)。p53表达与五年无瘤生存率不相关。结论COX多变量分析显示bcl-2表达和淋巴结转移是独立的预测指标。

Expression of bcl-2 and metastasis of lymphnodes as independent prognostic indexes of cutaneous squamous cell carcinoma

Objective To investigate the effective prognostic indicators.Methods 32 pathological paraffin block of patients suffered from cutaneous squamous cell carcinoma were selected, and their size of tumor and metastasis status of lymphonodes were recorded;TUNEL dye was used combined with morphology to make statistics of the apoptosis index and expresses of p53.The status of survival in 1,2, 3,5years was followed-up;SPSS 11.0 was used to make statistics of the effective prognostic indicators. Results The survival rate(in 3,5years) was correlated to the lymphatic metastasis, grade of pathology(P= 0.026 and 0.037), and had no correlation with the size of neoplasm gross (P=0.076).The survival rate without neoplasms in 5 years did not related with expression of p53.Conclusion A multivariate Cox analysis shows that the expression of bcl-2 and the metastasis of lymphnodes are the independent prognostic indicators.
