| 摘 要： | 目的:探讨困难腹腔镜胆囊切除术(Laparoscopic cholecystectomy,LC)中逆行切除术的手术方法及临床意义.方法:采用逆行切除术的困难LC患者327例临床资料进行回顾性分析.结果:除中转开腹12例外,其余病人均LC成功.结论:困难LC适时选用逆行切除术,可提高腹腔镜手术成功率,减少胆管损伤等并发症的发生.

| Abstract: | Objective:To discuss the method of retrograde cholecystectomy in difficult laparoscopic cholecystectomy(LC) and its clinical significance.Methods:We reviewed 327 cases which underwent retrograde cholecystectomy from 2003 to 2008,summarized the causes of the operation difficulties and discussed the method and skills of retrograde cholecystectomy.Results:Most cases underwent successfully laparoscopic cholecystectomy except for 12 cases converted to laparotomy(3.67%);none had the bile duct injury,postoperative jaundice or hemorrhage but one with bile leak.Conclusion:Retrograde cholecystectomy could be chosen timely in difficult LC because it could increase the success rate and decrease the complications. |
