经裂孔切除术治疗食管癌和贲门癌

经裂孔食管切除术治疗１５例中晚期食管癌及贲门癌患者。结果１例术后死于肺炎，其余基本恢复正常进食，未发生重要医源性损伤和大出血。术中和术后平均总输血量７５０ｍｌ。较常见的并发症为喉返神经麻痹和颈部吻合口瘘。该手术不开胸，手术时间较短，对年迈、心肺功能差的病人非常有利。术前判断切除的可能性对选择合适的病例十分重要。

TRANSHIATAL ESOPHAGECTOMY IN HE MANAGEMENT OF ESOPHAGEAL AND CARDIAC CARCINOMA

rom July,1988,through April,1993,Transhiatal esophagectomy was performed on fif-teen patients who had carcinoma of the esophagus or gastric cardia. All were discharged withnear normal ingestion,except one patient died on the seventh postoperative day due to pneu-monia,There were no major iatrogenic injuries or massive hemorrhage. The average bloodtransfusion during perioperative period was 750 ml.The most common complications wererecurrent laryngeal nerve paralysis and transient anastomotic leaks,which occurred in 4 cas-es each.Transhiatal esophagectomy appears to be a safe alternative for intrathoracicesophageal malignancies at any level,preferably for old patients with impaired cardiac-pul-monary function. Preoperative staging is important in selection of candidetes for operation.
