微创经皮肾镜碎石与开放手术在小儿肾结石中的应用

目的探讨微创经皮肾镜碎石术与开放手术在小儿肾结石应用效果。方法按就诊顺序选取在笔者所在医院治疗的小儿肾结石患者180例,104例进行MPCNL治疗,76例进行手术治疗,对比两组患者手术前后血清肌酐浓度变化及手术时间、手术出血量、输血率、手术次数、住院时间、结石清除率、治疗总费用和并发症率等指标。结果 MPCNL组的手术时间、手术出血量、输血率、住院时间、并发症率明显少于手术组,但手术次数、治疗总费用和结石清除率的结果高于手术组,差异有统计学意义(均P<0.05)。术后4个月和8个月MPCNL组血清肌酐浓度明显低于手术组(P<0.05)。结论微创经皮肾镜碎石术治疗小儿肾结石临床疗效好,但费用和手术次数较多。

The contrast of minimally invasive percutaneous renal lithotripsy with open surgery in children with kidney stones application

Objective To explore the clinical effect between Minimally Invasive Percutaneous Nephrolithotomy and open surgery in pediatric renal calculus.Methods Orderly selected 180 cases of pediatric renal calculus patients treated in our hospital.104 cases were treated by MPCNL,76 cases were treated with open surgery.Compared to two groups with serum creatinine concentration changes before and after surgery,and the operation time,bleeding volume in surgery,blood transfusion rate,times of surgery,rate,postoperative hospital stay,calculi clearance,total treatment costs,complication rates and other indicators.Results Group MPCNL The operation time,bleeding volume,operation blood transfusion rate,postoperative hospital stay,complications rate was obviously less in the MPCNL group than that in the operation group,but the operation times,the total cost of treatment and calculi clearance was higher in the MPCNL group,the difference was statistically significant(P < 0.05).4 months and 8 months after operation,serum creatinine was lower in the MPCNL group than that in the operation group(P < 0.05).Conclusion MPCNL in the treatment of pediatric renal calculus has good clinical effect,but the cost and operation number was more.
