彩色多普勒伪像观察在监测输尿管导管位置中的应用价值

摘    要:目的探讨彩色多普勒超声引导下经皮肾镜碎石术(PCNL)中,观察彩色多普勒伪像监测输尿管导管前端位置的应用价值。方法行彩色多普勒超声引导下经皮肾镜碎石术44例次,应用超声灰阶模式和彩色多普勒伪像分别监测输尿管导管前端位置,并与肾镜直视下进行对照。结果 (1)44例次手术均成功,无一例发生严重并发症。(2)应用超声灰阶模式判断导管前端位置准确率56.8%(25/44);应用彩色多普勒伪像观察判断其准确率93.2%(41/44);两种方法准确率差异有统计学意义(P<0.01)。(3)通过观察彩色多普勒伪像可实时形象监测输尿管导管逆行灌注液流向。结论在灰阶超声的基础上,观察彩色多普勒伪像,可明确输尿管导管前端位置,监测逆行灌注水流流向,为通道建立及碎石工作的顺利进行提供帮助。

Abstract:Objective To investigate the value of color Doppler image artifacts in monitoring the localization of the anterior ureteral catheter in PCNL under the ultrasonic puncture. Methods Forty four cases were carried out PCNL under the puncture of color Doppler ultrasound.The localization of the anterior ureteral catheter was monitored by the ultrasound gray-scale and color Doppler image artifacts,and compared with nephroscope findings. Results(1) 44 cases were carried out PCNL successfully,and complication did not appear.(2) The localization of the anterior ureteral catheter was identified by gray-scale mode ultrasound(rate of accuracy was 56.8%,25/44) and the color Doppler image artifacts(rate of accuracy was 93.2%,41/44),the differences of two methods exist statistical significance(P<0.01)(3) By observing the color Doppler image artifacts can be monitoredreal-time water flow through ureteral catheter retroperfusion. Conclusion Observed the color Doppler image artifacts had important value in identifying the localization of the anterior ureteral catheter and monitoring real-time water flow through ureteral catheter retroperfusion.
