前列腺素E_1在冠状动脉搭桥手术中应用的研究

摘    要:目的观察前列腺素E1(PGE1)对冠状动脉搭桥术的心功能和氧合的改善。方法将60例拟行冠状动脉搭桥术的患者随机分为两组,治疗组(30例)在气管插管后即开始予0.9氯化钠+PGE1注射液10μg静脉滴注,1次/d,连续3 d,对照组(30例)予0.9氯化钠静脉滴注,1次/d,连续3 d。气管插管同时及手术结束后当时、术后次日、术后第3天,采集肺动脉压力(平均压mPAP),采集患者桡动脉血,检测氧分压(PaO2)、二氧化碳分压(PaCO2)。记录患者带呼吸机辅助呼吸时间,术后第5天,给予患者复查左室射血分数。结果治疗组患者在氧饱和度、PaO2、呼吸机辅助呼吸时间、肺动脉压力、术后心功能方面,均较对照组有了明显的改善。结论 PGE1对冠状动脉搭桥术患者的心功能和氧合方面都能起到改善的作用。

Abstract:Objective To observe the improvement of prostaglandin E1 in OPCAB. Methods Sixty patients were randomly divided into two groups.30 cases in treatment group were treated by intravenous infusion of 0.9 sodium chloride and 10mg PGE1 injection,30 cases in control group were treated by intravenous infusion of 0.9 sodium chloride,once daily for 3d.Pulmonary arterial pressure(mPAP),PaO2 and PaCO2 were collected during and after tracheal intubation and at 1 day and 3 days after operation.We also observed the ejection fraction(EF) and mechanical ventilation time at 5 days after operation. Results The level of pulmonary arterial pressure,PaO2,PaCO2,EF and mechanical ventilation time after operation were significantly improved in the study group than in the control one. Conclusion PGE1 shows effective heart function and oxygenation improvement in coronary artery bypass grafting.
