CT灌注成像对脑膜瘤诊断分型的临床价值

摘    要:目的 探讨CT灌注成像对不同亚型脑膜瘤诊断和临床治疗的指导价值。资料与方法 对17例经手术病理证实的脑膜瘤患者进行CT血流灌注成像及常规MRI增强扫描,并根据病理分型将患者分为3组进行分析。CT灌注原始图像传至工作站经灌注软件处理获得时间-密度曲线及灌注参数。采用SPSS软件将灌注参数进行统计学分析。结果 各组脑膜瘤的脑血容量(CBV)、脑血流量(CBF)、平均通过时间(MTT)及血管表面通透性(PS)均明显高于对侧脑组织。纤维型、血管瘤型及上皮型脑膜瘤的CBV、CBF、PS间存在统计学差异,而各型脑膜瘤的MTT无显著性差异。结论 CT灌注成像能够定量地获取脑膜瘤血供信息,有助于对脑膜瘤的术前分型及治疗方案的制定。

Abstract:Objective To evaluate the value of CT perfusion in preoperative assessment of different meningioma subtypes.Materials and Methods 17 patients of meningiomas verified by surgery and pathology were underwent with conventional MR and multislice CT perfusion. All cases were divided into three groups according to the different pathological types of meningiomas. CT data were transferred to online working station and processed to obtain time density curves, color perfusion maps and perfusion parameters, including cerebral blood volume (CBV), cerebral blood flow (CBF), mean transit time (MTT) and permeability surfaces (PS) in tumoral parenchyma, necrosis and peritumor brain edema. Perfusion values were analyzed statistically by using SPSS software.Results The CBV, CBF, MTT and PS of meningiomas were significantly higher than those of contralateral normal brain tissue in each groups. Furthermore, a significant difference in CBV, PS and CBF was found in meningothelial, angiomathous and fibroblastic types of meningiomas, no significant difference in MTT was found in different types of mengingiomas.Conclusion CT perfusion can be served as a useful method to provide hemodynamic information both for differentiation of the subtype of meningiomas and management planning before operation.
