经腹腔镜胆囊切除术对内分泌及代谢的影响

目的 :对经腹腔镜胆囊切除术 (LC)对内分泌及代谢的影响进行对照观察。方法 :35例经腹腔胆囊切除术与 30例剖腹胆囊切除术分 3阶段进行对照观察血浆儿茶酚胺、可的松、C 反应蛋白 (C RP)等浓度变化。结果 :LCCO2 气腹过程中血浆肾上腺素、去甲肾上腺素未见升高 ,反而略有下降 (P >0 .0 5 ) ,而在剖腹胆囊切除术中升高明显 ,尤其在术后 4h ,C RP及IL 6均高于LC组 ,P <0 .0 1。结论 :LC对神经内分泌刺激轻微 ,因而 ,对代谢的影响亦轻。

The Effect of Laparoscopic Cholecystectomy on Endocrine and Metabolism

Objective:To study the agitation of endocrine and metabolism when laparoscopic cholecystectomy was done.Methods:35 patients undergoing laparoscopic cholecystectomy and 30 patients undergoing opening of abdomen and incision of cholecystectomy were examined.The changes of concentrations of catecholamine,cortison and C-reactive protein(C-RP) in plasm-body were observed.Comparison was divided into three phases.Results:The findings showed that during the laparoscopic procedure of pnumoperitoneum with CO 2 there was no increase of plasma adrenaline and norepinephrine.On the contrary,there was slight drop,P>0.05.However,in the case of cholecystectomy through abdominal incision,the increase was significant, P<0.01,especially four hours after operation,the C-RP and IL-6 levels were higher than those of laparoscopic group.Conclusion:The laparoscopic irritation to neurendocrine is slight,thus affecting metabolism slightly.
