初诊2型糖尿病胰岛素强化治疗120例临床分析

目的探讨早期胰岛素强化治疗对初诊2型糖尿病(T2DM)患者血糖控制达标的疗效。方法将重庆市渝北区人民医院120例初诊T2DM患者予胰岛素强化治疗,作为治疗组;另选取120例初诊T2DM患者予口服药物治疗,作为对照组。分别观察2个月后各组空腹血糖(FBG)、餐后2h血糖(PBG)、糖化血红蛋白(HbAlc)、胰岛素水平、C肽水平变化情况。强化治疗后随访1年,观察疗效。结果经胰岛素强化治疗后FBG、PBG、HbAlc等显著下降,空腹及餐后2h胰岛素水平、C肽水平,比对照组明显增高,差别有统计学意义(P<0.01)。结论胰岛素强化治疗组疗效明显优于对照组,能更有效地控制血糖水平,降低高糖毒性,减轻胰岛素抵抗,使胰岛细胞功能恢复。

Clinic Analysis of Intensive Insulin Therapy in 120 Newly Diagnosed Type 2 Diabedc Patients

Objective To study the effect of intensive insulin therapy for newly diagnosed type 2 diabetic patients in early time. Method Select 120 newly diagnosed type 2 diabetic patients randomly as treatment group, they were treated as intensive insulin therapy. At the same time, select another 120 newly diagnosed type 2 diabetic patients randomly as control group, they were treated by oral hypoglycemic drugs. Then, observe the changes in the fasting blood glucose（FBG）, postprandial blood glucose （PBG）, glycosylated hemoglobin Alc（HbAlc）, insulin level, C peptide level of the tow groups. Follow up the treatment group for one year. Result The levels of FBG. PBG and HbAlc were reduced significantly in intensive insulin therapy group. Fasting insulin level and C peptide level, postprandial insulin level and C peptide level in treatment group were higher than the control groups significantly, P〈0.01. Conclusion The effect of intensive insulin therapy were better than the control group, it did much better in control the blood glucose level, reduce the high glucose toxicity, reduce the insulin resist and making the islet cells function recovery.
