FFA与OCT对糖尿病性黄斑水肿临床诊断的对比与分析

摘    要:目的 探讨荧光素眼底血管造影(FFA)和光学相干断层扫描(OCT)在糖尿病性黄斑水肿(DME)临床诊断中的作用.方法 糖尿病视网膜病变(DR)Ⅰ~Ⅳ期患者70例126只眼,均进行FFA及黄斑区OCT检查,对DME诊断结果进行比较分析.结果 FFA诊断DME 88只眼(69.8%),OCT诊断DME104只眼(82.5%),二者诊断符合率为84.1%(106/126).DR Ⅰ期,OCT的DME检出率(45.8%,11/24)高于FFA(16.7%,4/24),差异有统计学意义(配对x2检验,x2=4.000,0.025<P<0.05);DRⅡ期、Ⅲ期、Ⅳ期,两种检查方法检出率差别无统计学意义(P>0.05).结论 OCT检查可直观反映视网膜的厚度及层次形态,对早期DME敏感性高,与FFA联合应用,对于DME患者的及早诊断、制定治疗方案,观察治疗效果有重要意义.

Abstract:OBJECTIVE To evaluate the diagnostic value of optical coherence tomography (OCT) and fundus fluorescence angiography (FFA) on diabetic macu!ar edema (DME). METHODS One hundred and twenty-six eyes of 70 patients, who were clinically diagnosed with diabetic retinopathy (stage Ⅰ-Ⅳ ), were taken OCT and FFA. The features of image by OCT and FFA were observed and the results were analyzed. RESULTS The detection rate for the diabetic macular edema was 69.8% with FFA and 82.5% with OCT, the consistency was 84.1%. There was a significant difference between the two diagnostic methods at DR stage I (Z2=4,0.025〈P〈0.05). CONCLUSIONS OCT could intuitively indicate macular thickness and structure, and had sensitive diagnosis features for diabetic macular edema at early stage. The combination of OCT and FFA had important implications for the diagnosis, treatment and following-up of diabetic macular edema.
