黄疸肝病患者一种白细胞异常直方图原因探讨

目的:探讨黄疸肝病患者应用K-4500血液分析仪检测白细胞计数出现一种异常直方图的原因。方法:白细胞计数出现异常直方图的黄疸肝病患者标本,同时用手工目视计数法进行白细胞计数,并对标本进行洗涤(去除黄疸血浆、白细胞、血小板),再应用K-4500检测白细胞,所得结果运用stata7.0进行统计分析,并同时与肝功能指标做分析。结果:出现异常直方图的黄疸肝病患者标本,K-4500计数结果明显高于目视计数法,P<0.001;应用K-4500检测洗涤红细胞(去除黄疸血浆、白细胞、血小板)稀释样本,所得单位红细胞的白细胞结果与原样本差异无统计学意义,P>0.05。出现异常直方图的病例与对照组比较,肝功能部分指标差异有显著性,P<0.001。结论:出现此种异常直方图的黄疸标本的白细胞计数,仪器法结果明显高于手工法。而这一原因主要是由于红细胞破坏不完全所致,且与肝脏代谢异常有关。

The Study of the Abnormal Histogram of White Blood cell in Patients of Liver Diseases with Jaundice

Objective To study the reason of the abnormal histogram of WBC in patients of liver diseases with jaundice detected by instrument(K-4500).Methods To detect WBC count of the specimen with abnormal histogram in patients of liver diseases with jaundice by K-4500 and manual,and detect the same specimen washed red blood cell(RBC)removed plasma、WBC、platelet,then analysed all results and compare with liver function test by stata7.0.Results WBC counts detected by K-4500 higher than it by manual evidently.No significant difference between WBC counts of diluted specimen removed plasma、WBC、platelet by K-4500 and original specimen,P>0.05.Significant difference was found on liver function test between original group and comparison group,P<0.001.Conclusion WBC counts detected by K-4500 was higher than it by manual in abnormal patients with jaundice.It dued to the incomplete destruction of RBC,and it was related with liver metabolize abnormity.
