大于胎龄儿产妇的妊娠期处理探讨

目的 探讨大于胎龄儿在预产期前终止妊娠是否能减少剖宫产率及对新生儿的损伤.方法 排除胎位不正的产妇,对145例大于胎龄儿产妇在预产期前引产、预产期后分娩自然发动和近42周引产组进行比较,同时按孕24～28周行50g葡萄糖筛选(50g葡萄糖粉溶于200～250 mL水中,15min内服完,1h后抽血测血浆血糖,用氧化酶法,测血浆血糖≥7.8mmol/L为异常),筛选结果分糖筛选正常组和异常组,比较其预产期前引产及预产期后分娩自然发动和近42周引产组的剖宫产率及新生儿预后.结果 观察组的剖宫产率与对照组无明显差别,新生儿评分、羊水污染在两者间也无明显差异.结论 对于大于胎龄儿在预产期前引产并不能降低剖宫产率.

To terminate pregnancy before expected date of confinement for large for gestation age

Objective: To investigate advantages of terminating pregnancy before expected date of confinement for gestation age(LGA).Mothods: 145 cases of pregnant women with LGA were induced labor before EDC,spontaneous delivery after EDC and induced labor about 40 weeks,these pregnant woman devided to two groups by the result of 50g glucose screening test,the rate of caesareant section and the prognosis of neonates.Rusults: There is no different in caesareant section rate,vaginal delivery rate,neonate Apgar scoring and the rate of ammiotic fluid stained among the observed group and controlled group.Conclusions: Artificial labors before expected date of confinement for pregnant with LGA has no effect in decreasing the rate of caesarean section.
