儿童后尿道狭窄手术径路的应用评价

目的 评价儿童后尿道狭窄各种手术径路的应用,提高儿童后尿道狭窄的治疗效果。方法 对24例儿单后尿道狭窄患者的手术径路进行回顾性分析。结果 19例(79.2％)手术治愈。采用单纯经会阴径路5例中治愈3例(60％);经会阴耻骨下缘径路10例中治愈9例(90％);经耻骨联合会阴径路9例中治愈7例(77.8％)。结论 儿童后尿道狭窄的各种手术径路均存在一定的优缺点。经会阴耻骨下缘径路具有操作方便,创伤小,并发症少,且不破坏骨盆稳定性等优点,尤其适合于儿童。

Evaluation of the surgical approach in the operation of posterior urethral stricture in children

Objective To present the preliminary experience on the operative approach in the surgical management of complex posterior urethral stricture in children. Methods The operative approach of 24 cases of posterior urethral stricture was retrospectively reviewed. Results The total success rate was 79.2% (19/24), 5 cases were operated with simple transperineal approach and the success rate was 60% (3/5); transperineal - inferior pubic approach was adopted in 10 and the success rate was 90% (9/10) ; transpubic- perineal approach in 9 with a success rate of 77.8% (7/9). Conclusion Each operative approach has its own advantages and disadvantages. Transpcrineal - inferior pubic approach is a valuable technique in clinic with its simple operation, minor traumatic, less complications and no pelvic instability.
