中枢神经系统感染患儿脑脊液蛋白质检测的临床意义

摘    要:目的 探讨脑脊液中不同蛋白质成分对中枢神经系统感染患儿的临床价值。方法 应用免疫透射比浊法,速率散射比浊法等测定47例中枢神经系统感染患儿脑脊液微量总蛋白(m-TP),微量白蛋白(m-ALB),免疫球蛋白(Ig)和补体C3、C4一系列蛋白质指标,并与非感染组对照。结果 非感染组各项指标均在正常范围,各组脑膜炎m-TP、m-ALB、Ig均有不同程度的升高,且化脓性脑膜炎与结核性脑膜炎组增加更明显,其中化脓性脑膜炎组以IgM升高更明显,结核性脑膜炎组以IgG升高最明显。化脓性脑膜炎与结核性脑膜炎组补体C3、C4阳性率显著高于对照组和病毒性脑膜炎组。结论 中枢神经系统感染患儿血脑屏障受到不同程度损害,各种蛋白成分的检测对三种脑膜炎的诊断和鉴别诊断有重要临床意义。

Abstract:Objective To study the clinical implication of various proteins in cerebrospinal fluid of the pediatric patients with central nerve system infeciton. Methods Micro total protein (m TP), micro albumin (m ALB), immunoglobulin (Ig) and complements C 3, C 4 in cerebrospinal fluid of 47 pediatric patients with central nerve system infection were detected by using immunoturbidimeter method and rate nephelometry.Results These proteins in non infection patients were within the normal range, but increased in each group of meningitis, especially in the groups of purulent meningitis and tubercular meningitis.The level of IgM was significantly higher in the patients with purulent meningitis than in the other two groups, while the level of IgG was significantly higher in the patients with tubercular meningitis than in the other two groups.The positive rate of C 3 and C 4 in the purulent meningitis groups and the tubercular meningitis groups was higher than in the control group and viral meningitis groups.Conclusion The detection of various proteins in cerebrospinal fluid might contribute to the diagnosis and differential diagnosis of 3 kinds of meningitis disease.
