腹腔镜去顶术、开放性手术和穿刺术治疗肾囊肿的临床比较研究

目的比较分析腹腔镜下去顶术、开放手术及穿刺术治疗肾囊肿的优劣点。方法对2004-2006年收治的161份肾囊肿相关手术病例（其中腹腔镜手术89例,开放性手术39例,穿刺术33例）进行统计分析。具体比较项目包括手术时间、手术切口长度、术中出血量、留置导尿管和引流管的时间、禁食时间、术后住院时间以及术后复发率等状况。结果腹腔镜手术和穿刺术在术中出血量、手术切口、留置导尿管和引流管的时间、禁食时间、术后住院时间等方面明显优于开放性手术,而腹腔镜手术组和开放性手术组术后复发率明显小于穿刺术,差别有显著的统计学意义（P〈0.01）。腹腔镜手术组和开放性手术组之间的手术时间没有显著差异。结论肾囊肿治疗上,穿刺术和腹腔镜手术具有手术创伤小、术后恢复快等优点,疗效上腹腔镜手术和开放手术具有优势,特别是腹腔镜手术对开放性手术和穿刺术具有综合优势。

The therapeutic effect of laparoscopic renal cyst unroofing and renal cyst puncture with open renal cyst unroofing

Objective To compare the therapeutic effect of laparoscopic renal cyst unroofing and renal cyst puncture with open renal cyst unroofing. Methods The clinical date of 89 cases treated by laparoscopic renal cyst unroofing and 39 cases receiving open renal cyst unroofing and 33 cases receiving renal cyst puncture were retrospectively analyzed. Results Operation time of laparoscopic and open renal cyst unroofing had no significant different. The amount of bleeding during operation,the indwelling catheter time, the drainage tube time,the fasting time, the hospital stay were all better than the open surgical(P<0.01).Conclusions The laparoscopic renal cyst unroofing has the advantages of less trauma and pain and rapid recovery,and it is better than the open renal cyst unroofing.
