顺逆行穿带锁髓内钉治疗肱骨干骨折疗效比较

为比较带锁髓内钉治疗肱骨干骨折不同穿钉法的优缺点,以提高肱骨干骨折的治疗水平.对从肩峰处大结节内侧向远端顺行穿钉治疗的27例(A组)和从鹰嘴窝上向近端逆行穿钉治疗的15例(B组)的疗效进行了比较分析.结果显示两组患者全部骨性愈合,但逆行穿钉法手术并发症少,基本不存在功能障碍,明显优于顺行穿钉法.认为逆行穿钉避免了对肩袖的损伤,对肩肘关节活动无明显影响,肩肘关节功能恢复快.

A THERAPEUTIC-EFFFCT COMPARISON OF RETROGRADE AND ANTEROGRADE LOCKING PINNINGS FOR HUMERAL SHAFT FRACTURE

In order to compare the advantages and shortcomings of different intramedullary pinning methods for treating humeral shaft fracture and to improve the therapeutic level of humeral shaft fracture, 27 cases (Group A) were treated by anterograde pinning (distad from the medial side of the acromial greater tubercle) and 15 ones (Group B) by retrograde pinning (proximad from the olecranon fossa ); the therapeutic effects of the two groups were observed and compared. The results showed that all cases of the two groups obtained bony healings; but Group A had an obvious advantage over Group B in the shoulder-elbow function, little surgical complications and no basic dysfunction. It was believed that the retrograde pinning avoids the injury to the rotator cuff and speeds up the function recovery of the shoulder and elbow with no significant interference with the shoulder and elbow.
