腹腔镜直肠癌根治术后吻合口漏的危险因素分析

目的:分析腹腔镜直肠癌根治术后吻合口漏的危险因素.方法:回顾性分析中日友好医院2009年1月～2012年6月间115例腹腔镜直肠癌根治术患者的临床资料,以手术后是否发生吻合口漏为因变量,对患者临床资料进行单因素及多因素非条件Logistic回归分析.结果:本组患者吻合口漏的发生率为5.2％(6/115),回归分析显示吻合口平面距肛缘≤5cm、体重指数≥25kg/m2、手术时间≥270min是吻合口漏发生的独立危险因素,相对危险度分别为10.909、7.911、8.553.结论:吻合口平面距肛门≤5cm、体重指数≥25kg/m2、手术时间≥270min的患者易发生吻合口漏.

Analysis of risk factors for anastomotic leakage after laparoscopic radical resection on rectal cancer

Objective:To study the risk factors for anastomotic leakage(AL)after laparoscopic radical resection of rectal cancer(Dixon).Methods:The clinical data of 115 cases undergoing laparoscopic rectal resection in the hospital from January 2009 to June 2012 were reviewed retrospectively.Results:The incidence of AL was 5.2%(6/115).Univariate analysis and multivariate logistic analysis regression revealed anastomotic level(AV)≤5cm,body mass index(BMI)≥25kg/m2,surgical time≥270min were the independent risk factors for anastomotic leakage after laparoscopic radical resection of rectal cancer(Dixon)(OR=10.909,7.911,8.553).Conclusion:AV≤5cm,BMI≥25kg/m2,surgical time≥270min can increase the incidence of AL.
