中枢神经系统恶性黑色素瘤的诊断及治疗

目的：研究中枢神经系统黑色素瘤的临床病理特点，探讨诊断与治疗的方法。方法：分析1994年10月-2000年8月收治的9例颅内与脊髓黑色素瘤的诊断与治疗。结果：临床表现无特异性，症状、体征及影像学检查难以明确诊断。9例患者中1例肿瘤位于脊椎，1例位于鞍区，3例位于小脑脑桥角（CPA）区，4例位于大脑凸面。7例术前误诊或示明确诊断。随访9例，8例死亡，1例恢复正常生活。结论：中枢神经系统恶性黑色素瘤发病率低，误诊率高、预后差。恶性黑色素瘤应在手术的基础上采取综合治疗。

The Diagnosis and Treatment of Central Nervous System Malignant Melanoma

Objective:To investigate the diagnosis and tre atment of CNS malignant melanoma.Methods:The diagnosis and treatment of9ca ses of cranial and spinal malignant melanoma were reviewed.Results:CNS melan oma can't be definitely diagnosed according to symptoms,signs and medical ima ges.1case located in spinal and one case in sella region.3cases situated in CPA region and4cases in cerebral hemisphere.7cases was misdiagnosed or can't be diagnosed definitely.5cases was followed up and only one was alive.Conclu sion:CNS malignant melanomas are extremely rare tumors of the central nervous system.Misdiagnosis rate remain at high level and prognosis is poor.Combined t herapeutic means should be adopted on the basis of operation.
