主动脉夹层的螺旋CT诊断评价

目的探讨螺旋CT对主动脉夹层的诊断价值。方法回顾分析23例主动脉夹层的CT表现，重点观察真假腔、内膜瓣、破口、病变范围及附壁血栓等。结果根据DeBakey分型，Ⅰ型13例，Ⅱ、Ⅲ型各5例。23例全部显示真假腔和内膜瓣，显示破口8例，附壁血栓17例。3例出现左侧胸腔积液，1例出现心包积液。结论螺旋CT能清楚显示主动脉夹层的真假腔、内膜瓣及附壁血栓，是诊断主动脉夹层的重要检查手段。

Evaluation of spiral CT in dissection of aorta

Objective To evaluate the application of spiral CT in diagnosis of aorta dissection. Methods 23 patients with aorta dissection were retrospectively analysesed. The true lumen, pseudocoele, endomembrane valve, crevasse, extent of disease and mural thrombosis were observed. Results According to the DeBakey grouping, the numbers of patients of type Ⅰ , Ⅱ , and Ⅲ were 13, 5, and 5, respectively. True lumen, pseudocoele, and endomembrane valves were displayed in all the 23 patients. Crevasses were found in 8 patients and mural thrombosis in 17 patients. Conclusion In dissection of aorta, the true lumen, pseudocoeles, endomembrane valves and mural thrombosis could clearly be discovered with spiral CT, which is an important exam in diagnosis of aorta dissection.
