声门下吸引气管切开导管在神经外科ICU中的应用

目的比较声门下吸引气管切开导管与普通气管切开导管的临床使用效果。方法将我院神经外科监护病房116例气管切开患者分观察组和对照组各58例。观察组采用声门下吸引气管切开导管吸痰,对照组采用普通气管切开导管吸痰,对两种方法的肺部感染率、平均吸痰时间、护理工作量进行对比研究。结果观察组和对照组吸痰的次数分别是10.2次和14.6次,P<0.01、吸痰的量分别是50.2ml和45.7ml,P<0.05、肺部感染发生率均分别是46.55%和65.52%,P<0.05,差异均具有统计学意义。结论声门下吸引气管切开导管吸痰效果优于普通气管切开导管吸痰效果,能降低肺部感染率,减少吸痰次数,从而减轻护士的工作量。

Application of attracting subglottic tracheotomy cannula in neurosurgery ICU

Objective To compare the clinical effects of attracting subglottic tracheotomy cannula and ordinary tracheotomy cannula. Methods 116 hospitalized patients in our neurosurgery ICU were divided into observation group(n=58)and control group(n=58).Patients in observation group received sputum aspiration under glottis in tracheotomy cannula,and patients in control group received sputum aspiration in ordinary tracheotomy cannula,for comparative study about the lung infection rate and average suction times and nursing workload of two ways. Results In observation group and control group,the suction times were 10.2 and 14.6 respectively(P<0.05),the measures of sputum aspiration were 50.2ml and 45.7ml respectively(P<0.05),and lung infection rates were 46.55% and 65.52% respectively(P<0.05).All the differences were significant. Conclusions The effect of subglottic tracheotomy cannula is superior to that of ordinary tracheotomy cannula in sputum aspiration.Suction under glottis can decrease lung infection rate and sputum aspiration times,so it can reduce the workload of nurses.
