护理综合干预对呼吸机相关性肺炎的影响

目的探讨降低呼吸机相关性肺炎发生率的有效措施。方法将2009～2011年110例在笔者所在医院呼吸科使用呼吸机时限>48h的患者分成实验组和对照组。每组各55例。对照组给予常规护理,即患者采取自动卧位,常规饮食;实验组给予热湿交换器、严格饮食和体位管理、每日评估能否撤机或拔管、营养支持和应用免疫调节剂等。对两组患者情况进行比较,比较两组使用呼吸机的患者VAP发生率和气管插管时间。结果两组VAP的发生率和气管插管时间差异显著,差异有统计学意义(P<0.05)。结论采用护理综合干预措施能够降低VAP的发生率和缩短插管时间。

The influence of integrated care intervention on the incidence of ventilator-associated pneumonia

Objective To explore the effective measures on reducing the incidence of ventilator-associated pneumonia.Methods 110 cases in the respiratory department from 2009 to 2011,in which the duration of mechanical ventilation is more than 48 hours,were divided into experimental and control groups,with 55 cases in each group.The control group received routine care of automatic postural and conventional diet.Meanwhile,the experimental group was given the heat and moisture exchange,restricted diet,postural management,nutrition support,immunomodulatory agents and daily assessment of the feasibility of weaning or extubation.The incidences of VAP and tracheal intubation time of the two groups were compared.Results The incidences of VAP and tracheal intubation time of the two groups were significantly different,with statistical significance(P < 0.05).Conclusion Integrated care interventions can reduce the incidence of VAP and shorten the duration of intubation.
