19例主动脉夹层的诊治分析

目的加深对主动脉夹层的临床特点及诊治的认识,降低其误诊和病死率。方法对主动脉夹层患者的一般资料、检查结果、治疗方法、病情转归情况作回顾性分析。结果首次误诊7例,手术5例,介入治疗8例,保守治疗2例,术前死亡4例,术后死亡1例。结论主动脉增强CT检查是确诊主动脉夹层主要手段;控制血压和心率是药物治疗的首要问题,根据夹层分型和高危因素确定治疗方案。

Analysis of diagnosis and therapy in 19 patients with aortic dissection

Objective To decrease the misdiagnosis and fatality rate by enhancing awareness of clinical features and diagnoses of aortic dissection.Methods General status,results,treatments and prognoses of patients with aortic dissection were retrospectively analyzed.Results The mistake diagnosis number of first consultation was 7 cases.5 patients were treated with operation.Through inter-ventional therapy,8 cases obtained obvious effect.2patients were cured by conservative treatments.Before operation 4cases died,2 patients died postoperatively.Conclusions Aortic enhanced CT is a major method for the diagnosis of aortic dissection.The main factor of drug therapy is to control blood pressure and heart rate.Treatment depends on high risk factors and classification of AD.
