摘 要: | 目的探讨偏头痛(migraine)患者脑干听觉诱发电位(brainstem auditory evoked poten-tial,BAEP)的变化及危险因素。方法纳入52例偏头痛患者和50例健康对照者,均行BAEP检查。结果偏头痛患者BAEP异常率为71.15%,左侧Ⅴ波潜伏期(latency period,LP):偏头痛组为(5.65±0.29),对照组为(5.51±0.19),两组间差异有统计学意义(t=2.628,P=0.011);右侧Ⅲ波潜伏期:偏头痛组为(3.77±0.20),对照组为(3.68±0.16),两组间差异有统计学意义(t=2.050,P=0.044);右侧Ⅴ波潜伏期:偏头痛组为(5.67±0.23),对照组为(5.54±0.16),两组间差异有统计学意义(t=2.927,P=0.005);左侧Ⅲ~Ⅴ峰间期(interpeak latency,IPL):偏头痛组为(1.90±0.22),对照组为(1.78±0.21),两组间差异有统计学意义(t=2.354,P=0.021);右侧Ⅰ~Ⅲ峰间期:偏头痛组为(2.14±0.19),对照组为(2.03±0.12),两组间差异有统计学意义(t=2.502,P=0.014);右侧Ⅰ~Ⅴ峰间期:偏头痛组为(4.06±0.36),对照组为(3.89±0.16),两组间差异有统计学意义(t=2.206,P=0.030)。二分类logistic回归分析显示,性别[优势比(odds ratio,OR)=0.078,95%可信区间(confidence inter-val,CI)0.009-0.697;P=0.022],持续时间[优势比(odds ratio,OR)=1.121,95%可信区间(confi-dence interval,CI)0.991-1.191;P=0.041]与BAEP独立相关。结论偏头痛患者急性发作期脑干某些区域存在神经电生理改变,性别及头痛持续时间是其独立危险因素。

Abstract: | Objective To investigate the changes of brainstem auditory evoked potential(BAEP) and risk factors in migraines during attack.Methods 52 migraines and 50 controls were chosen.All were then examined by BAEP.Results 71.15% of migraines with abnormal BAEP.The significant prolongation of III and V wave latencies of BAEP was found in migraine patients only.Left V wave latency of migraines was 5.65±0.29 and control was 5.51±0.19(t=2.628,P=0.011);right III wave latency of migraines was 3.77±0.20 and control was 3.68±0.16(t=2.050,P=0.044);right V wave latency of migraines was 5.67±0.23 and control was 5.54±0.16(t=2.927,P=0.005).The significant prolongation of Ⅲ-Ⅴ,Ⅰ-Ⅲ and Ⅰ-Ⅴ interpeak latencies of BAEP was found in migraine patients only.Left Ⅲ-Ⅴ wave interpeak latency of migraines was 1.90±0.22 and control was 1.78±0.21(t=2.354,P=0.021);right Ⅰ-Ⅲ wave interpeak latency of migraines was 2.14±0.19 and control was 2.03±0.12(t=2.502,P=0.014);right Ⅰ-Ⅴ wave interpeak latency of migraines was 4.06±0.36 and control was 3.89±0.16(t=2.206,P=0.030).Binary logistic analysis showed that the gender(odds ratio = 0.078,95% confidence interval 0.009-0.697;P=0.022) and duration(odds ratio = 1.121,95% confidence interval 0.991-1.191;P=0.041) were independently correlated with BAEP.Conclusions Neurophysiological changes of certain areas of the brainstem were found in migrainers during attack,which were independently correlated with gender and duration. |
