高血压中医证型与颈动脉粥样硬化的相关性研究

目的探讨高血压中医证型与颈动脉粥样硬化程度之间的关系,揭示其内在的规律,以此判断患者的病情及预后,为临床提供客观指标,以指导临床诊疗。方法对207例高血压患者进行中医辨证分型及颈动脉B超检查,对比各证型颈动脉内-中膜厚度(IMT)、斑块发生率的情况。结果各证型颈动脉IMT比较,依次为痰湿壅盛型>阴虚阳亢型>阴阳两虚型>肝火亢盛型,其中痰湿壅盛型与其余3型比较均有显著性差异(P均<0.01)。痰湿壅盛的斑块发生率明显高于其他证型(P<0.05)。结论高血压患者不同中医证型颈动脉硬化程度不同,高血压痰湿壅盛型是颈动脉粥样硬化的高危证型,从痰论治是中医临床治疗高血压致颈动脉粥样硬化的重要治则。

Study on correlation between TCM Syndrome types of hypertension with atherosclerosis

Objective It is to approach the relationship between TCM Syndrome type of essential hypertension and the extent of Carotid Atherosclerosis,reveal its built-in laws,and judge the patient's condition and prognosis,in order to provide objective clinical indicators to guide clinical treatment.Methods 207 cases with essential hypertension were differentiated and the carotid artery was detected by B-ultrasound.The carotid carotid Intima-Media Thickness(IMT) and incidence of plaque of different TCM Syndrome type were compared.Results The IMT of different TCM Syndrome type with a difference,that were excessive accumulation of phlegm-dampness type> Yin deficiency with Yang hyperactivity type> Yin and Yang deficiency type> hyperactivity of liver fire type,and there were significant differences between excessive accumulation of phlegm-dampness type and the remaining three types(P<0.01).The incidence of plaque of excessive accumulation of phlegm-dampness type was significantly higher than that of other types(P<0.05).Conclusion The degrees of carotid arteriosclerosis in essential hypertension patients with different TCM Syndrome type are different,excessive accumulation of phlegm-dampness type is a high risk factors of carotid atherosclerosis.Treated by Resolving Phlegm is an important method in treatment for carotid arteriosclerosis with hypertension in TCM.
