原发性乳腺肿瘤104例的高频声像图特征与病理对照分析

摘    要:目的 分析乳腺良恶性肿瘤高频二维声像图特征并评价其在鉴别诊断方面的价值。方法 104例乳腺原发性肿块按其高频二维声像特征诊断为良性和恶性,检查结果与病理检查对照。结果 乳腺良恶性肿瘤的二维声像特征存在一定重迭,特定的恶性肿瘤具有较多的良性特征。二维超声诊断的恶性符合率、良性符合率以及总符合率分别为73.5%,85.5%和79.8%。结论 二维声像图特征是超声诊断和鉴别诊断乳腺良恶性肿块的基本依据,但较小的及“中间型”的乳腺肿瘤需要使用彩色多普勒、频谱多普勒以及能量多普勒辅助诊断。

Abstract:Objective To evaluate the features of breast neoplasm on high frequency 2 dimensional ultrasonography and their value for differentiating benign or malignance. Methods 104 primary breast lumps, including 55 benign changes and 49 malignant tumors, were diagnosed to benign or malignance according to its characteristics on 2 dimensional ultrasonography. The result of ultrasonography was contrasted to pathology. Results 73.5% of malignance and 83.5% of benign changes were correctly diagnosed, the total coincidence was 79.8%. There is a certain overlap between the features of benign and malignance on ultrasonography. Compared with the benign lumps with malignant features, a few more malignant tumors appear as benign on ultrasonography. As a result, the malignant coincidence is lower than benign's. Conclusion The 2 dimensional ultrasonographic feature of breast neoplasm is the basis of diagnosis with ultrasonography. Color Doppler flow imaging, pulsed wave Doppler and power Doppler imaging should be helpful auxiliary when neoplasm is quite small and unidentifiable.
