肺表面活性物质治疗新生儿呼吸窘迫综合征的临床疗效

摘    要:【目的】观察肺表面活性物质(PS)治疗新生儿呼吸窘迫综合征(NRDS)的疗效。【方法】对本院115例NRDS患儿在给予机械通气治疗的基础上分为两组,治疗组(71例)加用PS治疗,对照组(44例)未加用PS。观察比较治疗前后两组患儿的临床疗效。【结果】治疗组在应用PS后患儿呼吸困难、呻吟、三凹征、发绀症状减轻或消失。用药1h后以及24h后,复查血气分析示动脉氧分压(PaO2)升高、二氧化碳分压(PaCO2)下降、pH值由略偏低变为正常,与治疗前及对照组相比差异均有显著性(P〈0.05)。用药6~8h后,复查胸片示治疗组肺部病变显著改善比例为50.7%,48h后复查胸片,治疗组肺部基本正常者为90.1%,均明显高于与对照组,两组相比较差异均有显著性(P〈0.05)。【结论】PS治疗NRDS疗效较显著,能有效改善患儿肺通气、换气功能。

Clinical Efficacy of Pulmonary Surfactant for the li-eatment of Neonatal Respiratory Distress Syndrome

Abstract:[Objective] To observe the efficacy of pulmonary surfactant(PS) for the treatment of neonatal respiratory distress syndrome(NRDS). [Methods] Totally 115 pediatric patients with NRDS receiving mechan- ical ventilation treatment were divided into two groups. The treatment group( n = 71) additionally received PS treatment. Control group( n =44) didn't received PS treatment. The clinica,1 efficacy of two groups were ob- served and compared between before and after treatment. [Results] The symptoms such as dyspnea, groa- ning, three-concave sign and cyanosis in pediatric patients of treatment group after PS treatment were allevia- ted or disappeared. Te reexamination of blood gas at 1 and 14 hours after administration showed that PaO2 in- creased and PaCO2 decreased, and pH value became normal from slightly low, and there were significant differences compared with before treatment and control group( P 〈0.05). At 6 to 8 hours after treatment, the reexamination of chest X-ray showed that patients with the improvement of lung lesion in treatment group occupied 50.7%. At 48 hours after treatment, the reexamination of chest X-ray showed that the patients with the normal lung occupied 90.1% which was obviously higher than that those in control group, and there was significant difference between two groups( P 〈0.05). [Conclusion] PS(Curosurf) for the treatment of NRDS has significant efficacy. It can effectively improve lung ventilation and exchange function.
