腹腔镜联合胆道镜下辅助小切口治疗胆管结石20例分析

[目的]探讨腹腔镜辅助小切口联合胆道镜在胆总管探查取石术中的疗效和体会.[方法]回顾分析本院2010～2011年20例双镜辅助下小切口胆总管探查取石术患者的临床资料.[结果]胆管结石20例,年龄40(16～75)岁,合并胆管炎5例,合并肝内胆管结石3例.20例均手术成功.T管术后14 d拔除,平均手术时间150 min,平均住院时间18d,胆总管均无结石残留或明显胆瘘.[结论]腹腔镜辅助小切口联合胆道镜在胆总管探查取石术中的应用,其操作简便,创伤小,临床效果好,适于基层医院推广应用.

Analysis of Bimirror Assisted-small Incision for the Treatment of 20 Patients with Bile Duct Stones

[Objective]To explore the experience of laparoscopic assisted-small incision combined with choledochoscope in common bile duct exploration and choledocholithotomy. [Methods]Clinical data of 20 patients undergoing bimirror assisted-small incision choledochotomy with exploration and choledocholithotomy from 2010 to 2011 in our hospital were analyzed retrospectively. [Resultsl Among 20 patients with bile duct stones aged 40 years old（16~75 years old）, 5 patients were complicated with cho!.an- gitis and 3 patients were complicated with intrahepatic bile duct stone. All patients were operated successfully. T-tube was removed 14d after operation. The mean operation time was 150min, and the average hospital day was 18d. No residual stone or obvious bile leak of common bile duct occurred. [Conclusionl Laparoscopic assisted-small incision combined with choledoehoscope used in cho- ledochotomy with exploration and choledocholithotomy is simple and minimally invasive, and has good clinical efficacy. Therefore it is suitable for promotion and application in primary hospitals.
