摘    要:【目的】探讨后路双侧开窗髓核摘除椎体间Cage植骨融合内固定治疗单节段腰椎间盘突出症的临床疗效。【方法】回顾总结2007-2010年收治的39例单节段腰椎间盘突出症经后路双侧开窗髓核摘除椎体间Cage植骨融合内固定术的临床资料,根据JOA评分和影像学结果,评价临床疗效。【结果】所有病例术后随访6个月至3年,平均2.5年按JOA评估标准,术前JOA评分(11.79±2.36)分,末次随访时JOA评分(23.61±2.28)分,改善率68.7%。骨性融合率89.7%。【结论】腰椎间盘突出髓核摘除,同时行椎间Cage融合后路内固定,可促进椎间融合并以维持椎间隙稳定,疗效满意。

Bilateral Posterior .Discectomy by Fenestration, Interbody Cage Bone Graft, Fusion and Internal Fixa- tion for the Treatment of Single-segment Lumbar Disc Herniation

Abstract:[Objective] To explore the clinical efficacy of bilateral posterior discectomy by fenestration, in- terbody Cage bone graft, fusion and internal fixation for the treatment for single-segment lumbar disc hernia- tion. [Methods] Clinical data of 39 patients with single-segment lumbar disc herniation undergoing bilateral posterior discectomy by fenestration, interbody Cage bone graft, fusion and internal fixation from 2007 to 2010 were analyzed retrospectively. The clinical efficacy was evaluated by JOA score and radiological results. [Results]All patients were followed up for 6 months to 3 years with the average of 2.5 year after the surgery. By JOA evaluation criteria, the preoperative JOA score was 11.79± 2.36 points and the last follow-up JOA score was 23.61± 2. 28 points, and the improvement rate was 68. 7%. The bone fusion rate was 89. 7%. [Conclusion]Lumbar discectomy combined with interbody Cage fusion and posterior fixation simultaneously can promote interbody fusion and maintain intervertebral stability. The result is satisfactory.
