颅内压监测及侧脑室外流联合脑脊液置换在脑室出血中的临床应用

摘    要:目的探讨颅内压监测及侧脑室穿刺外引流在脑室出血的临床应用。方法患者被随机分成联合治疗组(31例)和对照组(29例),联合治疗组患者行侧脑室穿刺引流联合定期脑脊液置换术。对照组单纯行脑脊液置换术。所有患者分别在第1,2,3,7,10,14,21d同时采用腰穿测颅内压和无创颅压检测仪测颅压,另外予格拉斯哥评分评价临床效果。结果二组颅高压和临床效果比较差异有统计学意义,同一患者同时检测,二种监测颅压方法结果差异无统计学意义。结论侧脑室穿刺引流联合脑脊液置换术更有效减少颅内高压,改善预后,同时应用无创颅压监测与有创颅压监测有利于及时调整治疗方案。

Abstract:Objective To discuss the clinical effect of ventricular drainage and lumbar puncture in patients with cerebroventricuIar hemorrhage. Methods Totally 60 patients were divided randomly into the union treated group(n = 31)and the control group(n = 29). The union treated group was treated by ventricular drainage and cerebrospinal fluid replacement, the control group was only treated by cerebrospinal fluid replacement. In all cases, intracranial pressure monitoring with lumbar puncture and intracranial pressure monitoring on 1, 3, 5, 7, 10, 14 days, then all cases were evaluated by Glsgow coma scale. Results There were statistical significance between the union treated group and the control group in intracranial pressure and clinical effects, but there were no statistical significance between the two types of noninvasive intracranial pressure monitoring for every case. Conclusion The ways of treatments with ventricular drain- age and cerebrospinal fluid replacement is better than only treated by cerebrospinal fluid replacement for pa- tients with eerebroventricular hemorrhage. Cases should be carried out as timely as possible to patients after intracranial pressuremonitoring in order to improve therapeutic schedule and the prognosis.
