社区卫生服务中心与乡镇卫生院职工满意度的比较研究

摘    要:目的 探讨“互联网+医疗”背景下,社区卫生服务中心与乡镇卫生院职工满意度情况和影响因素,为两类机构“差异化”运营和“一体化”服务提供建议。方法 走访8家基层医疗卫生机构,比例抽样调查270名职工作为调查对象;设计新型的职工工作满意度量表,并检验量表的信效度;通过非参数检验、方差分析进一步探讨城乡机构职工满意度差异。结果 量表具有良好的信效度,满足测量需求;乡镇卫生院职工在薪酬、技术支持、岗位、职业发展4个维度和总满意度上的得分均高于社区机构职工,且差异具有统计学意义。社区卫生服务中心职工工作满意度的影响因素有移动信息技术采纳能力(F = 4.805,P = 0.002)、采纳方式与教育水平(F = 2.621,P = 0.042)、专业(F = 3.428,P = 0.013)的交互作用;乡镇卫生院职工工作满意度的影响因素有移动信息技术采纳能力(F = 3.399,P = 0.020)和采纳方式(F = 3.918,P = 0.022)。结论 医疗信息化建设减少了城乡医疗服务供给的差异,移动互联网已经与基层卫生工作紧密融合;乡镇卫生院对于技术支持、职业发展有更高的现实感知,从而显示出更高的满意度水平;职工的移动信息技术采纳能力越高,其工作满意度越高;社区卫生服务中心职工以面对面方式服务时满意度最高,而乡镇卫生院职工在复合方式和在线方式服务时满意度更高,一定程度上反映了数字化进程中,两类机构在服务环境、服务对象以及服务困境的差异。

Abstract:The aim of this study was to explore the employee satisfaction and influencing factors of communityhealth service center and township health center under the background of Internet plus medicine, and to provide suggestions for the differentiated operation and integrated services of the two types of institutions. Methods Eight primaryhealth care institutions were visited, and 270 employees in proportion sampling were investigated. A new employee job satisfaction scale was designed, and the reliability and validity of the scale were tested. Non-parametric test and variance analysiswere used to further explore the differences of employee satisfaction in urban and rural institutions. Results The scale hadgood reliability and validity, and met the needs of measurement. The scores of township health center employees in the fourdimensions of salary, technical support, post, career development and total satisfaction were higher than those of communityhealth center employees, and the difference was statistically significant. The influencing factors of the job satisfaction of thestaffs in the community health service center were the mobile information technology adoption ability (F=4.805, P=0.002), theinteraction of the adoption way with education level (F=2.621, P=0.042), and subject (F=3.428, P=0.013). The influencingfactors of the job satisfaction of the staffs in the township health center were the mobile information technology adoption ability (F=3.399, P=0.020) and adoption way (F=3.918, P=0.022). Conclusion The construction of medical information reducedthe difference of medical service supply between urban and rural areas. And township hospitals had higher realistic perception of technical support and career development, which showed higher satisfaction level. The higher the mobile informationtechnology adoption ability of employees, the higher their job satisfaction. The satisfaction of community health service centerstaffs was the highest when they provided service face -to -face, while the satisfaction of township health center staffs washigher when they provide service in composite and online ways. To some extent, the situation reflected the differences of service environment, service object and service dilemma between the two types of institutions
