子宫平滑肌瘤患者住院费用影响因素分析

目的 了解子宫平滑肌瘤患者住院费用水平，探讨其影响因素。方法 收集2016 - 2018年子宫平滑肌瘤患者的病案首页信息，采用单因素方差分析和多元线性回归分析方法对影响因素进行统计推断、利用贡献度对费用增长情况进行描述。结果 860例子宫平滑肌瘤患者的住院费用呈逐年上升趋势(F = 61.159,P＜0.001)，次均费用由2016年的13 838.17元上升至2018年的18 956.48元。不同年份、科室、手术方式、住院天数均是住院费用的影响因素。城市公立医院综合改革后，患者医疗费用结构变化较大。结论 继续运用价格的杠杆作用优化医疗服务价格体系，制定临床路径规范诊疗手段和缩短住院天数是控制子宫平滑肌瘤患者住院费用的重要措施。

Analysis the hospitalization expenses and influencing factors of patients with uterine leiomyoma surgery

To analyze the influencing factors of hospital expenses of patients with uterine leiomyoma. Methods Collected the first page information of medical records of patients with uterine leiomyoma from 2016 to 2018, One-way ANOVA, multiple linear regression analysis method and contribution degree were used for statistical inference. Results The inpatient expenses of 860 patients with uterine leiomyoma showed an increasing trend(P<0.001), and the average expenses per visit increased from 13838.17 yuan in 2016 to 18956.48 yuan in 2018. Different years, departments, surgical methods and days of hospitalization are the influencing factors of hospitalization expenses. After the comprehensive reform of urban public hospitals, the structure of patients’ medical expenses has changed greatly. Conclusion It is an important measure to control the hospitalization expenses of patients with uterine leiomyoma to optimize the medical service price system, formulate clinical pathway to standardize diagnosis and treatment methods and shorten the length of stay.
