紧密型医联体“院地”服务模式实践与成效——以四川大学华西医院为例

目的 探寻跨组织医疗服务的“院地”服务模式的实践特点和其关键成功因素，为紧密型医联体建设与发展提供依据。方法 跟踪调查华西医院与成都市成华区政府开展的“院地”紧密型医联体建设过程和方法，对华西-成华医联体 “N+1+N”联盟架构中转诊绿色通道、检验同质化、家庭医生团队、信息化支撑的多项服务进行分析评价。结果 紧密型医联体的成效集中在基层医疗、上下联动、患者满意度、双向转诊制度等多个方面。结论 紧密型医联体服务模式成效显著，能充分适应“大健康”战略要求，为深化医联体服务创新能力、发挥更深层次效用打下坚实基础。

Practice and effectiveness of "large hospital and primary hospital" service mode of integrated medical association: a case study on West China Hospital of Sichuan University

The study aimed to explore practical characteristics and related key success factors of "Hospital Site" service mode within the sector of cross-organizational medical services for purpose of providing evidences for construction and development of compact medical association. Results By following up the investigations on the construction process and special measures of "Hospital Site" compact medical association carried out by West China Hospital and Chengdu Province Chenghua District government, a series of in-depth analyses and evaluations of multiple services, containing referral green channel,inspection homogeneity, family-doctor team and information support within "N +1 +N" Coalition Structure of West China Chenghua Medical Association have been achieved. Result The successes of compact medical association lie in multiple dimensions, namely primary health care, up-to-down linkage, patient satisfaction and dual referral system. Conclusion Compact medical association mode has achieved remarkable results, which can fully meet the strategic requirements of "Big Health" and lay solid foundations in deepening the innovation capability of medical-association services and playing more profound role in future tasks.
