经筋微创疗法不同操作方案治疗膝骨关节炎疗效对比观察

目的探讨经筋微创疗法不同操作方案治疗膝骨关节炎患者的临床疗效,优选治疗方案。方法将2017年9月—2019年5月在中国人民解放军南部战区总医院治疗的144例早中期膝骨关节炎患者随机分为对照组、关节内治疗组和循经治疗组,每组48例。3组患者均进行相同健康教育,对照组口服塞来昔布胶囊0.2 g/次,每日1次,共7 d;关节内治疗组予关节内针刀镜及经筋针治疗1次;循经治疗组予关节内针刀镜及经筋针治疗、经筋针循经治疗各1次。于治疗前及治疗1,7,30 d后评估3组患者疼痛VAS评分、膝关节功能Lysholm评分及关节活动度(ROM)。结果3组患者治疗1,7,30 d后VAS评分均较治疗前明显下降(P均<0.05),关节内治疗组及循经治疗组治疗1,30 d后VAS评分均明显低于对照组(P均<0.05);对照组治疗7,30 d后Lysholm评分和ROM均明显高于治疗前(P均<0.05),关节内治疗组、循经治疗组治疗1,7,30 d后Lysholm评分和ROM均明显高于治疗前及同期对照组(P均<0.05),且循经治疗组治疗1,7,30 d后Lysholm评分和ROM均明显高于关节内治疗组(P均<0.05)。结论经筋微创疗法不同操作方案均可缓解早中期膝骨关节炎患者关节疼痛症状,改善关节功能及活动度,较常规治疗起效快、治疗效应持续时间长,且循经治疗的方案更优。

Comparative observation on the therapeutic effects of different different schemes of meridian-sinew minimally invasive therapy on knee osteoarthritis

Objective It is to explore the clinical effect of different schemes of meridian-sinew minimally invasive therapy on knee osteoarthritis and to optimize the treatment scheme.Methods 144 patients of knee osteoarthritis in the early and middle stages treated in General Hospital of Southern Theater Command of PLA from September 2017 to May 2019 were randomly divided into control group,intra-articular group and meridian-following group,each group had 48 cases.Based on the same health education,the patients in the control group was given celecoxib capsule 0.2 g per time,once a day,totally for 7 days,the intra-articular group was treated with intra-articular needle knife scope and acupuncture of meridian-sinew for one time,the meridian-following group was treated with intra-articular needle knife scope,acupuncture of meridian-sinew and acupuncture of following meridian-sinew.The pain VAS score,Lysholm score and range of motion of knee joint were evaluated before treatment and after treatment for 1,7,30 days.Results The VAS scores of the three groups after treatment for 1,7,30 days were lower than those before treatment(P<0.05),and the VAS scores after treatment for 1,30 days in the intra-articular group and meridian-following group were lower than that in the control group(P<0.05).The Lysholm scores and range of motion of knee joint after treatment for 1,7,30 days in the intra-articular group and meridian-following group were higher than that before treatment and in the control group at the same time(P<0.05),and the meridian-following group was higher than the intra-articular group(P<0.05).Conclusion The different schemes of meridian-sinew release therapy can relieve the pain symptoms,improve joint function and range of motion in patients with knee osteoarthritis in early and middle stages,which is faster and lasts longer than the conventional treatment,and the therapeutic regimen of meridian-sinew release therapy in knee and follow meridians is better.
