坚骨胶囊联合胫骨高位截骨和关节镜清理术治疗膝骨性关节炎疗效观察

目的观察坚骨胶囊与胫骨高位截骨术、关节镜清理术治疗膝骨性关节炎的临床疗效。方法将2015年2月—2018年1月沧州中西医结合医院治疗的80例膝骨性关节炎患者随机分为2组,研究组40例给予坚骨胶囊与胫骨高位截骨术、关节镜清理术治疗,对照组40例给予常规胫骨高位截骨术联合关节镜清理术治疗,对比2组患者术后12个月后的临床疗效以及关节功能、疼痛、膝关节内翻等指标的改善情况。结果研究组患者术后12个月后的显效率与总有效率均明显高于对照组(P均<0.05)。治疗后2组患者的HSS评分与SF-36评分均明显升高(P均<0.05),研究组患者的HSS评分与SF-36评分明显高于对照组(P均<0.05)。治疗后2组患者的内翻角度、VAS评分、Lequesne评分、WOMCA评分均明显降低(P均<0.05),研究组患者的内翻角度、VAS评分、Lequesne评分、WOMCA评分均明显低于对照组(P均<0.05)。治疗后2组患者的步速、步长、步频均明显增高(P均<0.05),双支撑相明显降低(P<0.05),研究组患者的步速、步长、步频均明显高于对照组(P均<0.05),双支撑相明显低于对照组(P<0.05)。结论坚骨胶囊联合胫骨高位截骨术、关节镜清理术治疗膝骨性关节炎在改善膝关节功能,缓解疼痛以及临床症状方面效果更好。

Observation of therapeutic effect of Jiangu capsule combined with high tibial osteotomy and arthroscopic debridement on knee osteoarthritis

Objective It is to observe the clinical effect of Jiangu capsule combined with high tibial osteotomy and arthroscopic debridement on knee osteoarthritis.Methods 80 patients with knee osteoarthritis treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from February 2015 to January 2018 were randomly divided into 2 groups,40 patients in the study group were treated with Jiangu capsule,high tibial osteotomy and arthroscopic debridement,40 patients in the control group were treated with conventional high tibial osteotomy and arthroscopic debridement.The clinical efficacy in 12 months after surgery and improvement of joint function,pain,knee varus and other indicators were compared between the two groups.Results The apparent efficiency rate and total effective rate of patients in the study group in 12 months after surgery were significantly higher than those in the control group(P<0.05).After treatment,the HSS scores and SF-36 scores of the two groups were significantly increased(P<0.05),and the HSS scores and SF-36 scores of the study group were significantly higher than those of the control group(P<0.05).After treatment,the inversion angle,VAS scores,Lequesne scores,and WOMCA scores of the two groups were significantly reduced(P<0.05),the inversion angle,VAS scores,Lequesne scores,and WOMCA scores of the study group were less than those of the control group(P<0.05).After treatment,the pace,length and frequency of walk of the two groups were significantly increased(P<0.05),and the double support phase was significantly decreased(P<0.05),the pace,length and frequency of walk of the study group were significantly more than those of the control group(P<0.05)and the dual-support phase were significantly lower than the control group(P<0.05).Conclusion Jiangu capsule can improve the therapeutic effect of arthroscopy and high tibial osteotomy on knee osteoarthritis.It was more effectively in improving knee joint function,relieve pain and clinical symptoms.
