常州农村社区35~70岁人群代谢综合征及其组分与高尿酸血症的相关性研究

摘    要:目的 探讨常州市农村社区成人代谢综合征及其组分与高尿酸血症的相关性。 方法 调查对象来自于2005-2008年全球城乡前瞻性流行病学研究(PURE)的基线数据。调查内容包括问卷调查、体格检查和实验室检测。 结果 共4413人纳入研究,平均年龄(51.54±9.43)岁,男性1922(43.55%)人,高尿酸血症784(17.77%)人。在调整年龄、性别、婚姻状况、收入水平、吸烟和饮酒等因素后,代谢综合征(OR=2.98,95%CI:2.50~3.56,P<0.001)、血压升高(OR=1.82,95%CI:1.53~2.16,P<0.001)、中心型肥胖(OR=2.82,95%CI:2.38~3.35,P<0.001)和TG升高(OR=4.68,95%CI:3.97~5.51,P<0.001)均与高尿酸血症相关;TG升高分别与血压升高和中心型肥胖存在相加交互作用,交互作用指数S分别为1.47(95%CI:1.08~2.01)和1.41(95%CI:1.07~1.84)。 结论 代谢综合征及其组分与高尿酸血症相关,TG升高和中心型肥胖、血压升高对高尿酸血症患病有相加交互作用。

Abstract:Objective To investigate the correlation between metabolic syndrome and hyperuricemia and the interaction of metabolic syndrome components on hyperuricemia. Methods The subjects were from the baseline data of global Prospective Urban and Rural Epidemiological Study (PURE) in Changzhou city. The survey included questionnaires, physical examination and laboratory monitoring. Results A total of 4,413 subjects were included in the study, with an average age of 51.54±9.43 years, including 1, 922 males (43.55%) and 784 hyperuricemia (17.77%). The multivariate logistic regression analysis showed that metabolic syndrome (OR=2.98, 95%CI: 2.50~3.56, P<0.001), elevated blood pressure (OR=1.82, 95%CI: 1.53~2.16, P<0.001), central obesity (OR=2.82, 95%CI: 2.38~3.35, P<0.001) and elevated TG (OR=4.68, 95%CI: 3.97~5.51, P<0.001) were all associated with hyperuricemia after adjusting for age, gender, marital status, income level, smoking, drinking and so on. Further interaction analysis showed that elevated TG was associated with additive interactions with elevated blood pressure and central obesity on hyperuricemia, and the S were 1.47 (95%CI: 1.08~2.01) and 1.41 (95%CI: 1.07~1.84), respectively. Conclusion Metabolic syndrome and its components are associated with hyperuricemia. Elevated TG with central obesity and elevated blood pressure have additive interactions on hyperuricemia.
