选择性支气管动脉栓塞术中不同栓塞方案治疗咯血50例疗效观察

目的 探讨选择性支气管动脉栓塞术中采用不同栓塞方案治疗咯血的效果.方法 将50例咯血患者采用随机数字表法分为对照组24例、观察组26例,对照组选择性支气管动脉栓塞术采用明胶海绵(GS)栓塞治疗,观察组选择性支气管动脉栓塞术在GS栓塞基础上增加聚乙烯醇(PVA)双重栓塞治疗,对比两组手术疗效.结果 临床疗效方面,观察组痊愈率为57.69％,治疗总有效率为100.00％,对照组痊愈率为25.00％,治疗总有效率为79.17％,观察组总有效率优于对照组,差异有统计学意义(Z=-3.139,P=0.002).结论 支气管动脉造影明确咯血患者的病灶区域,选择合适的栓塞治疗方案能够快速终止咯血症状,安全高效,因此值得推广及使用.

Effect of selective bronchial artery embolization in the treatment of hemoptysis

Objective To investigate the effect of selective bronchial artery embolization in the treatment of hemoptysis.Methods 50 cases of hemoptysis were randomly divided into control group of 24 cases,26 cases in the observation group.The control group used gelatin sponge selective bronchial artery embolization treatment,while observation group was treated with gelatin sponge plus polyvinyl alcohol (PVA) double embolization.The efficacy of the two groups were compared.Results In the clinical efficacy,the cure rate of the observation group was 57.69％,the total effective rate of the treatment group was 100.00％,the cure rate of the control group was 25.00％ and the total effective rate of the treatment group was 79.17％,which obviously,the treatment effect of the observation group was better than that of the control group (Z =-3.139,P =0.002).Conclusion The bronchial artery angiography clearly identifies the regional information of the patients with hemoptysis,and it is worthy of popularization and application to select the appropriate treatment plan for the treatment of hemoptysis.
