1%盐酸丁卡因凝胶在男性患者留置导尿术中的应用效果分析

目的 探讨1%盐酸丁卡因凝胶在男性患者留置导尿术中的应用效果.方法 选取209例需留置导尿的手术患者按完全随机分组的方法 分为观察组(n=107)和对照组(n=102).观察组采用1%盐酸丁卡因凝胶作为润滑剂行导尿术,对照组采用石蜡油作为润滑剂行导尿术,比较两组患者一次留置尿管成功率、全身麻醉前(T1)和麻醉苏醒期(T2)患者的血压、心率值以及躁动、疼痛评分.结果 两组患者的一次性置管成功率均较高96.26%(103/107)、93.14%(95/102),差异无统计学意义(P>0.05);T2期观察组患者的收缩压、舒张压(122.96±11.50)、(76.23±6.28) mmHg]显著低于对照组(146.92±8.92)、(89.48±4.33) mmHg],差异有统计学意义(P<0.001);T2期观察组患者的躁动评分、数字分级法(NRS)评分、Wong-Baker面部表情量表法(WBFPS)评分为(0.80±0.64)、(2.65±0.88)、(1.47±0.49) 分]与对照组评分(1.25±1.00)、(3.82±1.18)、(2.55±0.88) 分]相比较,差异有统计学意义(P<0.001).结论 在男性患者留置导尿术前应用盐酸丁卡因凝胶润滑尿道及导尿管,效果优于用石蜡油润滑尿道及导尿管.

Application effect of 1% tetracaine hydrochloride gel in male patients with indwelling catheterization

Objective To probe into the application of 1% tetracaine hydrochloride gel in male patients with indwelling catheterization.Methods The 209 patients with indwelling catheterization were completely randomized into two groups.The experimental group (n=107) used 1% tetracaine hydrochloride gel as lubricant for urethral catheterization.The control group (n=102) used paraffin oil as lubricant for urethral catheterization.One-time success rate of catheterization,blood pressure,heart rate,agitation and pain scores before general anesthesia (T1) and at awaking time of anesthesia (T2) were compared between the two groups.Results The one-time success rates of catheterization were both high for the two groups,96.26% (103/107) vs 93.14% (95/102);the difference was not statistically significant (P>0.05).The diastolic and systolic blood pressure of the experimental group (122.96±11.50),(76.23±6.28) mmHg]were lower than that of the control group (146.92±8.92),(89.48±4.33) mmHg] in the T2 phase;the differences were statistically significant (P<0.001).The agitation score,NRS score,WBFPS score of patients in the experimental group (0.80±0.64,2.65±0.88,1.47±0.49) were lower than those of the control group (1.25±1.00,3.82±1.18,2.55±0.88) in the T2 phase;the differences were statistically significant (P<0.001).Conclusions The lubrication effect of tetracaine hydrochloride gel in urethra and catheter before the application of indwelling catheterization was better than that of paraffin oil.
