脑转移瘤的CT诊断

摘    要:目的分析156例脑转移瘤的CT表现,以提高对其诊断水平.方法 156例脑转移瘤均经病理证实,其中普通扫描93例,增强扫描33例,普通扫描后行增强扫描30例.结果转移瘤单发67例,多发89例.498枚瘤体表现为结节状,134枚瘤体表现为环状,其中16枚瘤体见内壁结节.13例为室管膜下转移瘤,4例增强扫描后瘤灶数目增多.转移瘤的瘤周水肿与转移瘤大小无关,与转移瘤的部位相关.结论脑转移瘤有比较典型的CT表现,结合临床可对其做出较为准确的定性诊断,可为治疗和随访提供有价值的影像学依据.

Abstract:Objective To improve the ability in diagnosing cerebral metastases, CT appearances of cerebral metastases in 156 patients were analyzed. Methods The original focus of 156 cases were confirmed by pathology. 93 patients were examined with plain CT scans, 33 enhanced CT scans, 30 plain and enhanced CT scans.Results Of the 156 cases, 67 cases single-focus and 89 cases multiple -focus were observed. CT features of cerebral metastases appeared as nodule pattern in 498 metastases and ring pattern in 134 metastases, including 16 metastases with nodules in cyst wall. 13 cases showed subependymal metastases. The increasing of metastases were found in 4 cases after enhanced CT scan. The peritumoral edema showed the correlation with the location of metastases and had no relationship with the size of metastases. Conclusion The CT appearances of cerebral metastases were characteristic. By using clinical features, CT images may determine the character of cerebral metastases and evaluate the effect of treatment.
