COPD患者血清PCT、hs-CRP和PA检测的临床意义

摘    要:目的探讨血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)和前白蛋白(PA)在慢性阻塞性肺疾病(COPD)患者中的检测价值。方法选取本院62例COPD患者为研究对象,另外选择20例健康体检者作为对照组,分别检测血清PCT、hs-CRP和PA水平,观察COPD患者病情与上述指标的关系。结果血清PCT和hs-CRP水平在COPD急性加重期高于缓解期,缓解期水平高于对照组(P〈0.05);而血清PA水平急性加重期和缓解期低于对照组,急性加重期水平低于缓解期(P〈0.05)。血清PCT和hs-CRP水平随着肺功能分级的升高而升高,而相反,PA水平则逐渐降低,差异有统计学意义(P〈0.05)。结论 PCT、hs-CRP和PA均参与了COPD气道炎症反应,且其表达水平与疾病严重程度相关,联合检测血清PCT、hs-CRP和PA,对判断COPD的病情严重程度和肺功能受损程度有重要临床意义。

Abstract:Objective To study the determination value of serum levels of procalcitonin(PCT), high-sensitivity C-reac- tive protein(hs-CRP) and prealbumin(PA) in patients with chronic obstructive pulmonary disease(COPD). Methods 62 cases of patients with COPD were chosen as study objects, and 20 healthy person in the same period were selected as control group, and serum levels of PCT, hs-CRP and PA were detected to observe the relationship between the indexes above and the severity of COPD. Results Serum levels of PCT and hs-CRP in patients with acute exacerbation were higher than those in remission, while those makers in remission also higher compared with healthy control group, the difference was statistically significant (P〈0.05);and the serum levels of PA in acute exacerbation was lower compared with remission, while the level of PA in remission was lower than that of healthy control group. Following the improve- ment of lung function, the serum levels of PCT and hs-CRP were increased(P〈0.05);in contrast, as the improvement of lung function, the level of PA was gradually reduced(P〈0.05).Conclusion PCT, hs-CRP and PA are involved in airway inflammation of COPD, and there is relationship between their expression levels and the severity of disease, it is of great significance to understand the severity of COPD and the extent of damage in lung function from detection of serum PCT, hs-CRP and PA.
