31例胫腓骨节段性骨折的手术治疗体会

目的探讨内固定术联合外固定术治疗胫腓骨节段性骨折的效果。方法选取2009年12月～2012年4月本院收治的31例胫腓骨节段性骨折患者作为研究对象,7例开放性骨折在清创后行有限内固定术联合外固定术治疗,24例闭合性骨折在肿胀消退后先行复位,然后根据情况行交锁髓内钉内固定术或外固定术治疗,观察上述31例患者的疗效。结果 7例开放性骨折患者中有1例因肢体毁损严重而截肢,2例合并外固定支架钉道感染,其余4例均获得满意治疗,治疗12个月后,痊愈3例,良好2例,一般1例,差1例。24例闭合性骨折患者均获得满意治疗,治疗12个月后,痊愈17例,良好5例,一般2例,差0例。结论内固定术联合外固定术治疗胫腓骨节段性骨折可以获得较好的疗效,尤其是对于闭合性胫腓骨节段性骨折,该方案的疗效更佳。

Surgical treatment experience of 31 cases of segmental fractures of tibiofibula

Objective To investigate the therapeutic effects of internal fixation combined with external fixation for seg- mental fractures of tibiofibula. Methods 31 cases of patients with segmental fractures of tihiofibula admitted to our hospital from December 2009 to April 2012 were selected as study objects.Among these patients,7 cases of open frac- tures were treated by limited internal and external fixation after debridement,24 cases of closed fractures were firstly implemented reduction of fracture after the swelling subsided,then intemal fixation by interlocking intramedullary nail or external fixation treatment according to the situation.The therapeutic effects of the 31 patients were observed. Re- suits Among 7 cases of open fractures,there was 1 cases of amputation because of seriously damaged limbs,2 cases of patients with nail way infection of external fixation support,and the other 4 cases were satisfactorily treated,12 months after treatment,with 3 cases of cure, 2 cases of well resuh,1 case of ordinary result,and 1 case of poor result.All the 24 cases of patients with closed fracture were satisfactorily treated,12 months after treatment,with 17 cases of cure,5 cases of well result, 2 cases of ordinary result, and 0 cases of poor result. Conclusion The internal fixation combined with external fixation in treatment of segmental fractures of tibiofibula can achieve good therapeutic effects,and better re- sults especially for closed segmental fractures of tibiofibula.
