闭合复位微创股骨近端髓内钉治疗老年股骨粗隆部骨折

目的：探讨应用闭合复位微创股骨近端髓内钉（PFN）治疗老年股骨粗隆部骨折的疗效。方法：应用PFN治疗老年股骨粗隆部骨折25例。其中，男12例，女13例；年龄59～93岁，平均70．5岁。按Evans股骨粗隆骨折分型标准，Ⅰ型2例，Ⅱ型4例，ⅢA型3例，ⅢB型5例，Ⅳ型8例，逆粗隆型3例。结果：25例患者经6-18（平均13）个月随访．所有患者获得骨性愈合。根据Harris髋关节评分，优16例，良7例，差2例。结论：PFN治疗老年股骨粗隆部骨折具有手术创伤小、固定可靠、可早期功能锻炼、并发症少等优点，是治疗老年股骨粗隆部骨折的理想方法。

Effect of closed reduction microtrauma and thigh-bone proximal end intramedullary nailing(PFN)in the treatment of old age intertrochanter fracture of femur

Objective：To observe the clinical efficacy of closed reduction mierotrauma and thigh-bone proximal end intramedullary nailing in the treatment of old age intertrochanter fracture of femur. Methods： 25 cases of old age intertrochanter fracture of femur treated with closed reduction mierotrauma and thigh-bone proximal end intramedullary nailing were collected. There were 12 men and 13 women in the 59-93 year old and the average age is 70.5 in the cases. There were 2 Ⅰ cases, 4 Ⅱ cases, 3 ⅢA eases,5 ⅢB cases, 8 Ⅳcases and 3 inverse tuberosity eases by Evans standards of intertrochanter fracture of femur. Results： 25 cases were observed in 6 to 18 months and the average is 13 months and every one was bony union. There were 16 excellent cases, 7 good cases and 2 bad cases by Hafris femoral articulation score standards. Conclusion：Closed reduction microtrauma and thigh-bone proximal end intramedullary nailing in the treatment of old age intertrochanter fracture of femur had the minimally invasive surgery, fixed, reliable early exercise and fewer complications advantages and was an ideal way to cure old age intertrochanter fracture of femur.
