CT导向穿刺活检在骨骼病变诊断中的价值

摘    要:目的探讨CT导引活检在骨骼病变的临床应用价值.方法对52例骨骼病变行CT导引下活检,每例常规取材2~3次.29例病灶区或灶周硬化明显者应用了Ackermann骨钻针,17例骨质破坏明显者应用了核心切割针,6例囊性或有明显液化区的病灶则细抽吸针.结果活检的诊断准确率90.4%(47/52).5例假阴性者包括2例增生硬化性病灶,3例液化性.结论 CT导引下骨骼活检是诊断骨病变重要手段,是一种安全、准确而有效的方法,对临床治疗决策具有重要的指导价值.

Abstract:Objective To evaluate the value of percutaneous bone puncture biopsy under CT guidance in bone diseases. Methods Percutaneous bone puncture biopsy was performed under the guidance of Siemens Somatom Plus 4 CT in 52 cases. The tissues in each case were collected routinely for 2 or 3 times. Ackermann needle, core cutting needle, and fine suction needle were used in 29 cases with obvious lesions or sclerotic lesions, 17 cases with obvious destruction of bone, and 6 cases with cystic lesions or lesions with obvious liquefied areas, respectively. Results Histopathological diagnosis was obtained in 47 patients. The accuracy of biopsy was 90.4%. The specimen was false-negative in 5 patients, including 2 cases of sclerotic lesion and 3 cases of liquefied lesion. Conclusion Percutaneous bone puncture biopsy, a safe, reliable, and important method for the diagnosis of bone lesions, is of great significance in clinical therapeutic decision-making.
