左炔诺孕酮宫内缓释系统及子宫内膜电切术治疗功能失调性子宫出血的临床研究

目的:探讨并比较左炔诺孕酮宫内缓释系统(曼月乐)及子宫内膜电切术(TCRE)治疗功能失调性子宫出血的临床效果。方法:98例患者中48例放置曼月乐环,50例选择TCRE,记录3～12个月PBAC评分,血红蛋白水平变化,不良反应及患者满意度。结果:1年后,曼月乐使用者月经量减少达89%,患者满意度为84%;选择TCRE者月经量减少达87%,患者满意度为85%。曼月乐使用者及选择TCRE者血红蛋白水平较放置前分别增加12.5%和13.2%(P<0.05)。结论:两种方法治疗功能性子宫出血均疗效确切,放置曼月乐环对操作者技术要求低,无手术风险,避孕效果安全、持久。

Clinical study on levonorgestrel-releasing intrauterine system and transcervical resection of endometrium for dysfunctional uterine bleeding

Objective:To explore and compare the therapeutic effect of levonorgestrel-releasing intrauterine system(LNG-IUS) and transcervical resection of endometrium(TCRE) on dysfunctional uterine bleeding.Methods:Among 98 patients with dysfunctional uterine bleeding,48 cases adopted LNG-IUS and 50 cases underwent TCRE.PBAC score,hemoglobin levels,adverse reaction and satisfaction degree of patients were recorded for 3～12 months.Results:At the end of 1 year,the menstrual blood volumes decreased to 89% and 87% of those before treatment in LNG-IUS group and TCRE group,the satisfaction degrees of patients were 84% and 85% in LNG-IUS group and TCRE group.The hemoglobin levels increased by 12.5% in LNG-IUS group and 13.2% in TCRE group(P<0.05).Conclusion:Both LNG-IUS and TCRE are effective in treatment of dysfunctional uterine bleeding,but placement of LNG-IUS requires less skills of operators,has no operative hazards and the therapeutic effect is safe and persistent.
