HELLP综合征11例母婴结局及分娩方式的探讨

目的探讨11例HELLP综合征对母儿结局的影响及分娩方式的探讨。方法对海淀区妇幼保健院11例HELLP综合征患者的临床资料进行回顾性分析,观察母婴并发症、孕产妇和围产儿死亡率;不同分娩方式的围产儿结局。结果 HELLP综合征的发病率为0.563‰,占同期妊娠期高血压疾病的3.3%。其母婴主要并发症有肾功能不全,胎盘早剥、产后出血、DIC、早产、胎儿窘迫、死胎等,共发生11例,发生率为100%。无孕产妇死亡,死胎2例,无新生儿死亡,发生率为18.2%,1例未进围产期,围产儿死亡率10.0%。剖宫产率占90.9%。结论 HELLP综合征母婴并发症多,围产儿死亡率高,应及时诊断,积极终止妊娠,以保障母亲安全,并降低围产儿死亡率。

Studies of 11 cases with HELLP syndrome in maternal- neonatal outcome and delivry mode.

Objective ： To investigate the maternal - neonatal outcome of 11 cases with HELLP syndrome and to discuss the deliv- ery mode. Methods ： 11 cases of HELLP syndrome were collected retrospectively in BeiJing Haidian Maternal ＆ Child Health Hospital. The Maternal and Newborn Complications, maternal and perinatal mortality and outcomes of perinatalbabies with different delivery mode were analyzed. Results： The total incidence of HELLP syndrome was 0. 563‰, and it was 3.3% in the patients with hypertensive disorders in pregnancy. The major complications were renal insufficiency, placental abrnption, postpartum hemorrhage, DIC, premature delivery, fatal distress, fetal death, etc. The incidence of fetal death, perinatal mortality and cesarean section rate in those patients were 18.2% , 10. 0% and 90. 9% respectively. Conclusion： HELLP syndrome have more complications, with a high perinatal mortality. Once diagnosis of HELLP is confirmed pregnancy should be terminated as soon as possible.
