传染性单核细胞增多症患儿细胞因子与T细胞亚群动态变化及临床意义

摘    要:目的研究传染性单核细胞增多症(IM)患儿细胞免疫功能动态变化及其临床意义。方法采用ELISA法和流式细胞仪分别检测了38例生长发育正常的IM患儿急性期和恢复期血清IL-6、IL-8和外周血T淋巴细胞亚群,并与36例同龄健康儿童组比较。结果急性期IM患儿血清IL-6、IL-8、CD3、CD8明显高于对照组及恢复期(P〈0.01),CD4、CD4/CD8比值明显低于对照组及恢复期(P〈0.01),恢复期患儿血清CD4升高,IL-6、IL-8、CD3、CD8降低,与对照组比较无显著性差异(P〉0.05),CD4/CD8比值仍低于对照组(P〈0.05),但高于急性期(P〈0.01)。结论 EB病毒感染后机体细胞免疫功能异常是IM发病的关键,为IM患儿的免疫治疗提供了理论依据。

Dynamic changes of the cytokine level and T lymphocyte subtype in children with infectious mononucleosis and its clinical significance

Abstract:Objective:To study the dynamic changes of cellular immune function in children with infectious mononucleosis (IM) and its clinical significance.Methods:ELISA method and the flowing cell instrument were usted to determined the serum levels of IL-6,IL-8 and T lymphocyte subtype in the peripheral blood of 38 cases with IM,who was in acute and convalescent,They were compared with 36 healthy children.Results:At the acute stage,the serum levels of IL-6,IL-8,CD3,CD8 cells were greatly increased(P〈0.01),CD4,as well as the ratio of CD4/CD8 were significantly reduced in comprison with normal controls (NC) and the cnvalescent(P〈0.01,At the convalescent,the levels of IL-6,IL-8,CD3,CD8 were decreased,CD4 were elevated,compareing with NS there were no remarkable difference(P〉0.05),but the CD4/CD8 ratio was significantly lower than the NC(P〈0.05),its was significantly higher than the acut stage(P〈0.05).Conclusion:The abnormal function of humoral immunity and cellular immunity after EBV infection were the crux of the invasion of IM,The theory basis was provided to children with IM for the adoptive immunotherapy.
