二维及三维超声诊断胎儿颜面部少见或复杂畸形的价值

摘    要:目的评价二维及三维超声用于诊断胎儿颜面部少见或复杂畸形的价值。方法对接受产前系统超声检查的胎儿进行颜面部的二维超声常规检查,对二维超声检查怀疑有颜面部少见或复杂畸形的病例进行三维超声检查。对产前超声诊断此类畸形的胎儿进行产后随访,比较二维超声与三维超声对该类畸形部位及细节的显示情况。统计学分析采用卡方检验。结果二维超声与三维超声对胎儿颜面部少见或复杂畸形部位的诊断准确率分别为87.0%、92.4%,差异无统计学意义(P=0.226);对畸形细节的诊断准确率分别为82.6%、88.0%,差异无统计学意义(P=0.298)。三维超声对某些特定类型颜面部畸形的检出率更高。结论三维超声对胎儿颜面部少见或复杂畸形的诊断准确率与二维超声相比无明显提高,需将二维与三维超声优势互补,才能够提高诊断准确率。

Abstract:Objective:To determine the value of two-dimensional(2D) and three-dimensional(3D) ultrasonography in prenatal diagnosis of rare or complicated fetal facial abnormalities.Methods:Face of the fetus was examined by 2D ultrasonography.Those who were suspected of having rare or complicated facial abnormalities with 2D ultrasonography were examined with 3D ultrasonography.2D and 3D diagnosis was compared to pathological outcome.Using Chi-Square test to compare the accuracy of 2D and 3D ultrasonography for the diagnosis.Results:The accuracy of 2D and 3D ultrasonography to diagnose the site of fetal rare or complicated facial abnormalities was 87.0% and 92.4% respectively and there was no statistical difference between them(P = 0.226).The accuracy of 2D and 3D ultrasonography to describe these abnormalities in detail was 82.6% and 88.0% respectively and there was no statistical difference between them(P = 0.298).3D ultrasonography was more useful in diagnosing some special types of facial abnormalities.Conclusions:3D ultrasonography is not superior to 2D ultrasonography in the diagnosis of fetal rare or complicated facial abnormalities.Both 2D and 3D ultrasonography are important and should be combined in the diagnosis.
