彩色多普勒超声诊断围产期脐动脉血流异常的临床价值

目的探讨彩色多普勒超声诊断围产期脐动脉血流异常的临床价值。方法选择2004年3月～2009年3月间在本院接受超声检查并在本院住院分娩的围产期脐动脉血流彩色多普勒超声表现为异常的60例孕妇,并与同期脐动脉血流检测正常孕妇60例比较,分析影响孕产妇脐动脉血流异常的因素及其预后。结果与对照组相比,观察组脐带绕颈、脐带扭转、妊高征、糖尿病、胎儿畸形和胎儿宫内发育迟缓均明显增多(P0.05);观察组宫内窘迫、羊水污染、Apgar评分≤7的比例明显增高(P0.05),剖宫产比例明显高于对照组(P0.05);死产明显增多(P0.05)。结论围产期脐动脉血流异常是影响孕产妇和胎儿预后的重要因素;早期采用彩色多普勒超声检测围产期孕产妇脐动脉血流变化情况对妊娠预后有重要意义。

The clinical value of color Doppler ultrasound in diagnosing blood current abnormality of arteria umbillicalis in the perinatal period

Objective：To investigate the clincal value of color Doppler ultrasound in diagnosing blood current abnormality of arteria umbillicalis in the perinatal period.Methods：To analyze the factors and prognosis of blood current abnormality of arteria umbillicalis by comparing 60 pregnant women（the control group） who had ultrasonic inspection and got blood current normality of arteria umbillicalis and 60 pregnant women（the study group） with blood current abnormality of arteria umbillicalis in the perinatal pericd with the help of color Doppler ultrasound in our hospital from March,2004 to March,2009.Results：Comparied with those in the control group,circulor of umbilical cord,torsion funiculi umbilicae,pregnancy induced hypertension,diabetes me litus,felal anomaly and foetus intrau terine growth retardation of the study group increase obviously（P〈 0.05） ;the proportions of intrauterine embarrassment,amniotic fluid pollution,Apgar score less than 7 of study group are obviously higher than those of the control group,and the rate of uterine-incision delivery is higher than that of control group（P 〈0.05） and dead-birth is higher too（P〈 0.05）.Conclusion：blood current abnormality of arteria umbillicalis in the perinatal period is one of the major fators that affect parturient and foetus prognosis,so adopting color Doppler ultrasound earlier to surpervise the conditions of blood current abnormality of arteria umbillicalis in the perinatal period is important to prognosis.
