泮托拉唑预防肾移植术后消化系统并发症的临床观察

目的研究质子泵抑制剂泮托拉唑预防肾移植术后消化系统并发症的疗效及安全性。方法将64例肾移植患者随机分为泮托拉唑治疗组31例与对照组33例，治疗组于肾移植手术后预防性应用泮托拉唑40mg，1次／d，对照组术后不应用任何抗酸剂。观察术后消化道并发症及泮托拉唑的不良反应发生情况。结果泮托拉唑组术后第1、2、3、4周时消化系统并发瘟的发生率分别为122．6％、51．6％、19．4％、3．2％，对照组为315．2％、209．1％、103．0％、39．4％，两组比较有显著性差异（P＜0．01）；泮托拉唑组消化道出血发生率明显低于对照组（P＜0．01）；在应用泮托拉唑期间患者耐受性良好，无明显不良反应。结论泮托拉唑能够明显降低肾移植术后消化系统并发症的发生率并减少消化道出血，不良反应少，是可以防治肾移植术后消化道并发症的安全有效的质子泵抑制剂。

Application of Pantoprazole in preventing digestive complications after kidney transplantation

Objective To investigate the efficacy and safety of Pantoprazole in preventing digestive complications of renal allograft recipients. Methods 64 cases of renal allograft recipients were randomized divided into two groups.31 patients(pantoprazole group) had received pantoprazole 40 mg once daily for four weeks and 33 patients(control group) had no received pantoprazole.The digestive complications and bad reaction of pantoprazole were observed for four weeks in both groups.Results The occurrence rate of digestive complications in 1st,2nd,3rd,4th week after kidney transplantation were respectively 122.6%,51.6%,19.4%,3.2% in pantoprazole group and 315.2%,209.1%,103.0%,39.4% in control group.The difference between the two groups was markedly significant(P<0.01).The occurrence rate of alimentary tract haemorrhage in pantoprazole group was significantly lower than that in control group(P<0.01).Pantoprazole was well tolerated.Conclusion As a safe and efficacious proton pump inhibitor,pantoprazole can reduce the occurrence rate of digestive complications and alimentary tract haemorrhage of renal allograft recipients.
