免疫性血小板减少性紫癜患儿外周血sCD40L检测

摘    要:目的探讨可溶性CD40配体(sCD40L)在免疫性血小板减少性紫癜(immune thrombocytopenic purpura,ITP)患儿体内的变化及其在发病机制中的作用。方法用ELISA方法检测30例患者治疗前后及30例正常儿童外周血sCD40L的水平,同时检测他们外周血中的血小板抗体(PAIgG)水平。结果ITP患儿治疗前外周血sCD40L的水平比正常儿童显著增高(P〈0.05),治疗后sCD40L的水平较治疗前显著降低(P〈0.05),sCD40L阳性的患儿血小板抗体水平显著增高,血小板计数显著降低(P〈0.05)。血小板抗体与血小板数量呈负相关(r=-0.83,P〈0.05)。结论sCIMOL可能是ITP发病的重要原因之一,检测ITP患儿外周血sCD40L的水平对明确诊断和判断预后有重要的临床意义。为应用抗CIMOL单抗治疗ITP提供理论依据。

Abstract:Objective To investigate the alteration of sCD40L level in patient with immune thromb cytopenic purpura(ITP) and its possible role in the pathogenesis of ITP.Methods Peripheral blood level of soluble sCD40L and platelet-associated antibody(PAIgG)in 30 untreated and treated ITP patients and 30 normal controls were detected by enzyme linked immunosorbent assay(ELISA).Results The sCD40L levels were significantly higher in those with untreated ITP than that in those with normal controls(P<0.05).the sCD40L levels in those with treated ITP were significantly lower than in those with untreated ITP(P<0.05),the levels of sCD40L and the platelet counts were negative relation(r=-0.83,P<0.05),The PAIgG level was significantly higher in those with sCD40L positive ITP than that in those with sCD40L negative ITP.And the platelet counts were significantly lower in those with sCD40L positive ITP than that in those with sCD40L negative ITP(P<0.05).Conclusion The increased level of sCD40L may be the potential reason of ITP.Detection of peripheral blood may be helpful for the diagnosis of ITP and the application of anti-CD40L antibody will probably be a new method for the therapy of ITP.
